School Nurse Information ….. |

1. A registered nurse is available during regular school hours to meet the health needs of students and staff. 2. Indiana law requires that all students must have a completed immunization record on file, unless a religious objection has been filed. This must be completed within 20 days following enrollment. The State mandates exclusion from school until documentation is presented. A list of immunization requirements is on file in the clinic. 3. All Kindergarteners and “new” first graders must present a completed physical exam form from their doctor. If a physical has not been completed at time of registration, please notify the nurse of appointment date. 4. All medication (prescription and over the counter) must be brought to the clinic by an adult. Medication must be in the original bottle accompanied by a physician’s order. Parent signature is needed on prescription and all over-the –counter medicines. 5. According to health policies of ACS, any student with a temperature over 100F and/or at the nurse’s discretion. Students may return after 24hours without diarrhea, vomiting or fever. Treatment for strep throat and pink eye must be initiated for 24 hours prior to returning to school with a physician’s note. 6. Please make your school nurse aware of any allergies, medication changes or special health needs as soon as they occur. This will help better serve your child. 7. Vision screenings are performed on all students enrolled in first and third grade. Those students with suspected visual problems will be screened at the discretion of the nurse, parent and/or teacher. 8. Hearing screenings are performed by speech therapist on all students enrolled in kindergarten, first, and fourth grades. Screening again will take place if there are concerns for hearing loss. 9. Scoliosis screenings are performed on all fifth graders. 10. It is imperative to have up to date phone numbers and emergency information on file in the main office. If there are alternate names and phone numbers available we will have a better chance of reaching you in an emergency. 11. Head lice is not an illness but a common nuisance throughout the year. ACS has adopted a no live louse policy. A student will be excluded from school with live lice and or viable nits (at the discretion of the nurse or designated school personnel. Student are allowed one day excused absence to eradicate this problem. Student will need to be rechecked prior to returning to school. The school will refer excessive absences (due to repeated infestations) to the School Resource Officer for further action. School Nurses…. Make a Difference |
