Triennials

TRIENNIAL EVALUATIONS

All special education students are required to be reevaluated every three years. This does not mean re-tested every three years.   Testing should only be done if:

Please do not request testing because you want to see “where the student is” or how the student is progressing.   The student’s classroom performance is the best indicator.

At the ACR during the school year before the triennial is due, notify the parent that the triennial is due the following year.   The case conference committee should at that time decide whether re-testing will be necessary and document this in the discussion summary.

If the plan is to complete a reevaluation summary plan, contact your school psychologist well in advance. He/she will complete the waiver form and send it to you.   You will complete the waiver in a case conference meeting, preferably the ACR.   If the student’s triennial is due before the ACR, you may hold the ACR early or schedule two separate case conference meetings.   The parent will sign and date the reevaluation summary plan, checking the appropriate boxes.   Attach the white copy to the case conference report, send the gold copy to Psych. Services, give the parent the pink copy, and keep the yellow copy for the student’s school file.   *Please write firmly so that all copies are legible.

If the parent decides at this conference that they do want re-testing, they should check the appropriate boxes and still sign and date the form.   The reevaluation summary plan will still serve as the student’s triennial reevaluation.   Testing will be considered “additional” testing and will have a 60-day timeline, which began on the date the parent signed the waiver form.   Also at that conference, have the parent sign Permission for Evaluation (form 210) and complete a Social/Developmental History Update form (202-T).   The teacher-of-record will complete a Referral (form 209).   Send all to Psych. Services.   A case conference will be required within the 60-day limit to discuss the results of the testing.

If re-testing is planned for the triennial, all forms (referral, social/developmental update, and parent permission) should be received in Psych. Services at least 30 instructional days before the triennial is due.   A case conference may be held to discuss the results of the testing, or results may be discussed at the next ACR.   Send Notice of Intent to Conduct Triennial Evaluation (form 204) to the parent when you send the referral paperwork (at least 20 days before the triennial date).

As soon as possible after the beginning of the school year, each TOR will receive a list of students with triennial and ACR dates.   Please compare this list with your records and report any discrepancies to Sandy Cunningham.

Be kind to your fellow teachers… if you have students whose triennials are due within the first couple of months of the following school year, take care of them this year.

For Triennials and Early Re-evaluations of students who are currently receiving Special Education Services (all grade levels):

  1. Referral Form No. 209 (4 pages) - This form should be completed, signed and dated by the student's regular classroom teacher.
    • Please be sure to completely fill out all demographic information.
  2. Parent Permission for Evaluation Form No. 210 (1 page)
    • Be sure all items are filled out completely.   Parent must initial line stating that he/she received a copy of the Notice of Procedural Safeguards.   He/she must also sign and date the form after checking permission given or not given.
    • The Public Agency Representative should be the person who has explained the referral process to the parent(s).
    • Double check that both signature lines are signed and dated.
  3. If we (ACS) have not tested the student before
  4. Notification of Intent to Conduct Triennial Evaluation Form 204

All of the above-required forms (white copies only) should be sent together to Sandy Cunningham, Psychological Services, Administration Building.   Copies or carbons should be given to the parent and another set should be kept in the student's file at the building.

Forms should be acquired from Central Stores (except the Woodcock-Johnson).
